Investigation Summary
On November 14, 2005, Employee #1 was pinned against a burn bin when he inadvertently engaged a forklift's transmission as he was dismounting it. He did not lower the forks and the forklift moved forward and pinned him against the burn bin, where a fire engulfed and killed him. There were no witnesses to the accident, and a coworker found him deceased about 45 minutes since the last time they had visual contact.
