Investigation Summary
Employee #1 was operating the coiler on a wire mesh machine and was "curling off" the roll. This involved bending the ends of the wires so that they hooked into the roll of mesh to prevent the coil from unrolling. While doing this, the coiler rotated unexpectedly and pulled his hands into the in-running nip point between the roll of wire mesh and the pressure bar. He suffered an amputation and but did not require hospitalization.
