Investigation Summary
Employee #1 and a coworker had been hired to install new electrical service in a manufacturing facility. They were using a powered industrial truck and a Palletainer, a bin made for general storage, to access wiring on the ceiling. The coworker was operating the controls of the forklift. He raised Employee #1, who was standing on top of the Palletainer, to a height of approximately 20 ft. Employee #1 began pulling wires when he shifted his weight on the Palletainer, causing it to tip off the forks. Employee #1 fell about 15 ft to the floor and sustained multiple injuries, for which he was hospitalized. The Palletainer was not designed as an elevated work platform, nor was there means to secure it to the forks of the truck. Neither Employee #1 nor his coworker had been trained to operate powered industrial trucks.
