Investigation Summary
On January 27, 2005, Employee #1 was rigging several bundled bars, weighing 492 lbs, to an overhead crane. Some additional steel bars were located on roll conveyor that was adjacent to where Employee #1 was working. The conveyor unexpectedly ejected the bars, which rolled on top of Employee #1 and pinned him between the ejected bars and the bundled stock. Employee #1's neck was fractured and he died. The accident investigation revealed that there was no machine guarding or barriers present to secure the bars from ejection.
