Employee Fractures Arm in Machine

Employee #1 was assigned to remove damaged boards from pallets using a lead board remover. The stoppers on the machine were not working properly. He was trying to repair the machine by following instructions given by a coworker. Employee #1 did not speak English, and the coworker giving instructions was normally used as a translator by the company. It was not clear from the investigation if a manager of the company instructed the translator to order Employee #1 to try to fix the lead board remover. The translator resigned the same day the investigation was initiated, therefore, information available to clarify the issue was limited. The carriage of the machine moved forward while Employee #1's arm was inside the machine. Employee #1 sustained fractures to his left arm and was hospitalized. Employee #1 was not trained nor authorized to work on equipment or perform lockout/tagout procedures.
