Investigation Summary
On June 9, 2004, Employee #1 was attempting to start a John Deere, Model Number 770, grader while it was in gear in the reverse direction. The grader started then moved in the backwards direction. Employee #1 was run-over and crushed by a grader blade that was approximately 6 inches off the ground and the front wheel. He was killed. A detailed inspection revealed that the grader's normal starting procedures were by-passed due to a broken starter relay. The grader had been operated in this fashion for about four days prior to the accident due to the replacement starter relay being on order.
