Employee is killed after falling from roof

On January 28, 2004, an employee, a roofing construction worker, accessed a 3.5 to 12 pitch roof by stepping down on the roof from an upper story deck. Other coworkers on site stated that there was frost on the roof. The evidence indicated that the employee fell 21.7 ft from the roof to the ground and struck a concrete retaining wall on the ground level. No fall protection systems were being used at the time of accident. The employee was killed.
