Employee's Finger Is Amputated in Tube Bender

At approximately 3:30 p.m. on December 11, 2008, Employee #1, a machine operator, was operating a Clark and Lewis CL300 hydraulically powered tube bender. He was performing a setup and placed a 2.375-in. work piece in the tube bender. The bender was not guarded. He intended to switch the tube clamp to open but switched the tube clamp to close. His finger was crushed between the work piece and the machines tube clamp. He sustained an amputation of his finger tip and was hospitalized.
