Investigation Summary
At approximately 2:30 a.m. on November 11, 2008, Employee #1 was operating a Sheeter machine in a facility that performs graphic design work. While she was operating the machine, a paper jam occurred. As Employee #1 went to remove the jammed paper with her left hand, two of her fingers became pinched in the drive belt of the machine. Employee #1 was taken to Queen of Valley Hospital, where she was hospitalized and treated for the partial amputation of her middle and ring fingers on her left hand. The incident investigation revealed that the machine was energized and running when Employee #1 attempted to remove the paper jam. In addition, no hand extension tool for grabbing the jammed paper was used. The power supply to the machine should have been disconnected and the moveable parts of the drive belt mechanically blocked or locked to prevent inadvertent movement, which was consistent with the employer's procedures in place.
