Power press operator gets arm amputated while removing a stu

On November 4th 2008, at approximately 12:35 p.m., Employee #1 was injured while performing his duties as a power press operator/setup person at Adams Campbell Company. Employee #1 was reaching into the point of operation on an Aida 150 ton mechanical power press with a partial revolution clutch and two handed controls. Work pieces were being fed into the die with an automatic feeder and were being extracted with compressed air. Employee #1 was attempting to remove a stuck piece from the progressive die fitted to the press. As Employee #1 reached into the point of operation without safety blocks or extension tools, the press ram cycled without the employee touching the two handed controls. Employee #1 sustained a traumatic amputation of his right arm below the elbow.
