Employee Amputates Fingers in Hamburger Machine

On August 27, 2008, an employee was working with one of four Hollymatic hamburger patty machines. The employee stuck his hand from the bottom discharge of the Hollymatic hamburger machine to remove meat. While doing so, his fingers were amputated between the metal housing and a plastic piston. The hazardous machine parts of the Hollymatic hamburger patty machine were not guarded against accidental contact and control of hazardous energy, lockout tag out procedures were not used. The employee was hospitalized and treated for an amputation.
