Investigation Summary
At approximately 9:50 a.m. on August 21, 2008, Employee #1 was working at a recycling center for metal and plastic containers. At the time of the accident Employee #1 was cutting a piece of metal with an alligator shears. In the process Employee #1's right hand was caught by the machine resulting in the amputation of his right ring and middle fingers. Employee #1 was taken to a hospital for treatment and was released after eight days.
