Employee Is Pinned Between Machine Parts, Fractures Chest

At approximately 4:00 a.m. on August 8, 2008, Employee #1 was working as a machine operator in the "Mill Building" on the 24-in. "Slitter" machine. Employee #1 was manually feeding the starter strips of steel into a mandrel that reels the strips into one roll. A carousel with four arms rotated to receive the rolls of steel after being wounded on the mandrel. A coworker was controlling the rotation of the carousel arm but became distracted with his other work duties of weighing rolls of steel and did not communicate with Employee #1. While feeding strips with his back turned away from the carousel arms, Employee #1 became pinned between the mandrel and carousel arms. Employee #1 sustained a crushing injury to his chest and was hospitalized for twenty days.
