Investigation Summary
At approximately 6:45 a.m. on April 30, 2008, Employee #1, of General Sealants Incorporated, was inserting paper material into the adhesive size roller machine. Employee #1's right fingers contacted the in-running rollers of the machine. Employee #1's hand was crushed, and he suffered fractures to his right ring and middle fingers. Cal/OSHA determined that the incident was caused by the employer's failure to provide adequate machine guarding on the adhesive size roller machine. As a result, a serious citation T8 CCR 4187(a) was issued to the employer.
