Investigation Summary
At approximately 1:49 p.m. on May 19, 2008, Employee #1 was cutting a piece of phenolic tubing with an unguarded table saw (Model Number 270 and Serial Number 207270) at a facility where they fabricate industrial plastics. As he worked, his fingers contacted the saw blade, causing an amputation to his ring and middle fingers.
