Employee's Eye Is Injured by Flying Object

On April 23, 2008, Employee #1, a brush operator, of Ted Levine Drum Company, was working on changing a part to a heavy duty DeWalt (Model Number D28494) angle grinder. He dropped the grinder on his workstation, and it hit a wrench. The wrench popped up and struck him in the eye, causing a serious eye injury. There were no witnesses to the incident, but it was reported that Employee #1 did not unplug the grinder and he did not remove it from the area to take it to a changing area. In addition, he did not have the supervisor change out the wire cup brush, per the company policy. All employees with this job duty were interviewed at this worksite, in addition to the supervisor and the maintenance supervisor. No citations were issued to the employer after the investigation.
