Investigation Summary
On April 28, 2008, Employee #1 was repairing an air conditioning system of a restaurant in La Verne, CA. He climbed to the roof of the second story building via a fixed ladder and began working on the air condition vents. After completing his work, he started descending on the fixed ladder while carrying a plastic bottle in right hand. As he descended, he lost his grip on the ladder and fell 16 ft. He sustained multiple bruises to his chin, left hand fingers and ribs. Employee #1 was transported to a medical center, where he underwent treatment and was hospitalized.
