Employee Gets His Thumb and Index Finger Amputated with Saw

On February 29, 2008, Employee #1 was working alone and cutting special louvers, which is a waxed wood, to approximately three to four inch in length on a CTD Saw Cut-Off Saw. The saw's blade was adequately guarded. However the employer had placed an additional plastic guard around the saw with openings on both sides for pieces being worked on to go through. The openings in the side of guard measured at approximately five and a half (5.5) inches in height and distance from the cutting blade and six and half (6.5) inches in length. As Employee #1 was cutting the louver, his hand entered the plastic guard to cut the length of the desired piece, which was three to four (3-4) inches in length. His hand slipped and he suffered an amputation of the left thumb and index finger. The employer did not provide instruction to Employee #1 to keep hands and fingers outside of the area below the blade until the blade comes to a complete stop. Additionally, the employer did not provide training specific to the saw and louver pieces Employee #1 was cutting. Employee #1's supervisor was in the area, but did not witness the accident.
