Investigation Summary

On December 12, 2007, Employee #1, who had been working as a building services technician for approximately 18 years, was working alone at the entrance of a carwash, trying to determine the cause of a malfunction. The line to the pressure switch, which triggers the carwash, was severed near the guide rail. In order to ensure that this was the cause of the malfunction, Employee #1 stepped over the guide rail and stood on the line to determine if the carwash would start. As he turned to see if the carwash had started, he slipped on debris in the area and landed on his right hip, resulting in a right hip fracture. Employee #1's supervisor was in his office in Ontario, at the time of the incident.
