Investigation Summary
At approximately 9:40 a.m. on December 12, 2007, Employee #1 was working in the "Gas House", where a gasser machine filled individual cans with product and flammable hydrocarbon aerosol propellants and then sealed those cans. Employee #1 was attempting to prevent production line down time by reaching into the point of operation and removing a product out of the line before it created a problem. As he attempted to remove a can from an energized and running production line, his right hand pinky finger got caught and was crushed in the point of operation. The existing guards were ineffective in preventing workers from reaching into the point of operation while the machine was energized and running. Employee #1 was transported to Western Hand and Orthopedics, where he stayed for a few hours for treatment of a subsequent right pinky finger tip amputation. The employer reported the accident in a timely manner.
