Investigation Summary
At approximately 3:10 p.m. on November 3, 2007, an employee operating an electric pallet jack to move grocery store merchandise to and form storage racks to delivery trucks. The electric pallet jack lost its braking capabilities. The employee's right foot was crushed and pinned between the plastic bumper of the electric pallet jack and a metal post The employee's toes were amputated. The employee was hospitalized and treated for an amputation.
