Investigation Summary
On August 25, 2007, Employee # 1 was working on a Tenter Frame Machine, M: TG-300B, Fire Electronics, Daeger, made in Korea. The machine was located near the exit of the work area. Employee # 1 was cleaning the in-running rolls of the machine, while it was running. Employee # 1 was standing on the edge of the machine to clean the rolls with a green fiber sponge. He slipped, and his left hand was caught in the in-running rolls of the machine. Employee # 1's index, middle and ring fingers on the left hand were partially amputated. He was hospitalized. Employee # 1's supervisor was working with Employee # 1, but he was not in his direct view. The company's procedure for cleaning the rolls of the machine was to use a water hose from a distance. Employee # 1 stated that everyone used the green fiber sponge to clean the rolls and that it was a common practice at the worksite. The employer did not ensure the employee was provided with and trained in use of extension tools or other methods to prevent the entry of the employee's hands in the zone of danger of the in-running rolls.
