Investigation Summary
At approximately 5:00 p.m. on October 5, 2007, Employee #1, a machine operator, was greasing the gear of a machine when his finger was caught by the gears. The machine was not stopped nor the power source deenergized. Employee #1 sustained the amputation of the tip of his finger with no bone lost. Employee #1 was not hospitalized.
