Investigation Summary

On September 19, 2007 Employee #1 of Angeles Welding & Manufacturing Inc., was injured when the load he was moving with a bridge crane struck him on his chest and pinned him against a metal stand resulting in a chest compression. He was taken to a local hospital for treatment and hospitalized in there for 15 days due to injuries to his chest. Angeles Welding & Manufacturing Inc. has been in the business of steel fabrication over many years with its main office at 9747 S. Norwalk Blvd., Santa Fe Springs, California, and employed about 125 employees on its payroll at the time of the accident. Employee #1 was employed as a welder about 3 1/2 years with the employer before the accident occurred in the welding area of the fabrication shop. At the time of the accident the injured, Employee #1, was moving a welded unit of 2-hollow metal shells, size 16'L by 8'H, approximate weight 2500 pounds, with a bridge crane in bay #2 when the load swung towards him and pinned him against a metal stand at his back. He was moving the load with a hand held pendant control box when accident occurred. There was no other employee or coworker in the area assisting him at that time. According to his employer, there was enough room in the work area for the employee to move away from the load but he did not do so. Employee #1 stated during the interview that push controls on the hand held pendant box were malfunctioning at the time of the accident and in the past and nothing was done to abate the hazard.
