Investigation Summary
At approximately 7:10 a.m. on July 21, 2007, Employee #1 was operating an order picker, Raymond EASi-OPC30TT, Serial Number EASi-OO-AE-26089. He was placing and removing goods from storage racks. He was in isle Number 14 removing an order from the racks. Each rack is approximately 5 ft high and he was elevated, in the order picker, at the fifth level, approximately 25 ft. He had a lanyard attached to the picker and to the belt loop of his pants. He fell and his pants ripped. Employee #1 was hospitalized with injuries to his head, shoulder, arm, and a fracture to his left knee and ankle.
