Investigation Nr: 202474227
Event: 06/07/2007
Employee Is Injured on Conveyor Belt

At approximately 4:00 p.m., on June 7, 2007, Employee #1, a machine operator, was working at a cardboard manufacturer helping remove cardboard from the top stacker. The employee got on the conveyor belt to remove the cardboard. Employee #1 did not turn off the conveyor belt before beginning his work. The employee was aware of the work rules, and was trained in lockout tag out safety. Employee #1 walked onto the top stacker conveyor belt, the sensor tripped, and the load pusher was engaged striking the employee in the lower leg and right ankle. Two other employees assisted the Employee #1 by turning off the line and getting help from management. Management called Emergency Medical Response, and the employee was transported to Whittier Presbyterian Hospital suffering multiple fractures of his right ankle, requiring surgery.
