Investigation Summary
At approximately 7:30 a.m. on July 2, 2007, Employee #1, a shipping clerk, was taking inventory of materials in the warehouse. Employee #1 was climbing storage racks without fall protection. Employee #1 fell backward off the racks approximately eighteen feet and struck the back of his head and his back. Employee #1 was airlifted to County USC Medical Center. He was pronounced "brain dead" on July 6, 2007, as a result of head trauma. Employee #1 died.
