Investigation Summary
At approximately 1:30 p.m. on May 18, 2007, Employee #1 was working at a car wash. He was working alone, cycling a 2005 Ford Explorer through the car wash. He placed the car in reverse instead of park. He exited the vehicle and fell, and the vehicle rolled over his left leg. Employee #1 was hospitalized with a fractured leg.
