Employee Sustains Cerebral Contusion in Slip and Fall

At approximately 8:30 p.m. on April 9, 2007, Employee #1 was working as a night custodian at the Royal Oak Intermediate School in Covina, CA. Royal Oak Intermediate School was a public school in the Charter Oak Unified School District for educational activities at the secondary and junior high levels. Employee #1 had been working at Royal Oak Intermediate School for four years. He was using liquid stripper and a stripping machine to strip wax off the floor in the "Commons" student cafeteria. Although he was working with another employee, his coworker was not in the immediate area. Employee #1 was wearing the appropriate footwear, but when he stepped on the stripping liquid, he fell to the floor. His supervisor was not there at the time of the accident, but the coworker noticed that Employee #1's backside was wet. Employee #1 got up, resumed his activity, and completed his task with the assistance of the coworker. The next day, Employee #1 felt unwell and reported the accident. He transported himself to the Foothill Presbyterian Hospital in Glendora, CA. He remained in the facility for two days for observation only, and he was treated for a cerebral contusion.
