Investigation Summary

Event: 04/18/2007
Employee's Finger Is Amputated in Conveyor

At approximately 9:40 a.m. on April 18, 2007, Employee #1, a machine helper, was working with a coworker on a LOBO Shaper Machine, Serial Number 29C1122-06. Employee #1 was waiting for a door being worked on to come out of the machine. The doors are moved through the machine by a block conveyor system. At the end of the line, the conveyor enters the machine. An opening is created at that point, where the blocks composing the conveyor system split before entering the machine. The split that is created between the blocks is approximately 0.5-in. wide. Employee #1's glasses were foggy and he couldn't see and he had his hand on the conveyor. His left hand middle finger was caught in the split created between the blocks resulting in an amputation of his left hands middle finger.
