Investigation Summary
At approximately 2:40 p.m. on March 9, 2007, Employee #1 was performing a dado cut using a Delta Unisaw table saw, Serial Number 05F105796. The birch wood he was cutting was approximately 24-in. in long by 5.5-in. wide and 1-in. thick. He finished the dado cuts and picked up the piece of wood with his left hand to look at the cut. He dropped the piece of wood on the dado blades, which caused the wood to slam against the fence where his right hand was. The force of the wood smashing against the fence crushed his fingers. His right ring finger was amputated up to the second knuckle and suffered a laceration to his little and index fingers. Employee #1 was hospitalized.
