Employee's Fingers Amputated in Unguarded Extruder

At approximately 8:30 a.m. on March 1, 2007, Employee #1 was on the roof of building Number 4 above the extruder room. He was attempting to fix the deficient production output of the extruder machine. He stuck his hand into the unguarded hidden moving parts of the motor. He suffered an amputation of 4 fingers on his right hand and was hospitalized.
