Investigation Summary
On December 12, 2006, Employee #1 was aligning on a Dodge pickup truck. The truck was parked on a lift, but not elevated off the ground. In order to check the caster of the alignment, Employee #1's coworker asked him if he wanted the truck started. At the time, Employee #1 was lying on the ground directly in front of the front axle and between the front tires. After Employee #1 told his coworker to go start the truck, the coworker checked the manual transmission to verify that the gear was in neutral. According to Employee #1's coworker, he moved the shift handle side to side and thought that the gear was in neutral, but in fact the transmission was in first gear. The coworker also verified that the vehicle's parking brake was on, and then he depressed the clutch and started the engine. When the coworker let the clutch out, the truck unexpectedly lunged forward, passed through the tire bump stop on the lift, and struck and pinned Employee #1. Employee #1 was hospitalized treated for a shoulder injury and a fractured pelvis.
