Investigation Summary
At approximately 3:10 p.m. on November 9, 2006, Employee #1 was clearing a jammed cardboard sheet (25 in. by 45 in.) from a Flexo folder machine while it was operating. When he placed his left hand under the sheet on a kicker plate, his left ring finger became caught in a 1 in. round cavity on the kicker plate. Employee #1 was hospitalized for a partial amputation of the finger.
