Investigation Summary

Investigation Nr: 202473039
Event: 09/29/2006
Employee Lacerates Left Hand When Struck by Conveyor

At approximately 8:00 p.m. on September 29, 2006, an employee was working below a vertical conveyor, and its drive chain broke and dropped down approximately six feet. The employee moved backwards, but the conveyor hit him on his left hand, resulting in a 4-inch long cut with some bone loss. The employee was taken to the hospital and was released the same day after surgery to his left hand. Investigations revealed that the original 0.25-horse power drive motor had been replaced by a larger 2-horse power electric motor. This larger motor put approximately eight times the amount of original torque on the chain and sprockets of the conveyor belt, which led to its eventual break down on the day of the accident.
