Employee's Fingertip Is Amputated by Rollers

At approximately 6:15 p.m. on August 29, 2006, Employee #1 was a machine operator with Paper Pak Products. Paper Pak Products manufactured incontinence products. Employee #1 was splicing a roll of poly film on Diaper Machine #1. During the splicing process, the machine was turned on, however the in-running rolls near the splicing operation were not running. Employee #1 held the new roll of poly down with his left hand, in close proximity to the in-running rollers. He activated the rollers with his right hand. His left hand was pulled into the roller and the tip of his left ring finger was amputated.
