Investigation Summary
At approximately 11:00 a.m. on May 19, 2006, Employee #1, a temporary employee, was operating a Pioneer broaching machine Number 20, Model Number VT- 630B. Employee #1 was told to load each part (bolt) on the top of a push-rod in the machine through an 8-in. by 9.5-in. opening on the housing of the machine. To make the parts, Employee #1 was supposed to press two palm buttons with his hands, which would cause the push-rod to move upward into the opening of operation where the was to be made. After the part was made, it was removed from the machine and Employee #1 was supposed to engage a lever and the then press the palm-buttons to descend the push-rod. Employee #1 stated that he pressed the palm buttons to descend the rod seconds before the accident he removed his hands off the palm buttons to load a part onto the push-rod. This caused the rod to move upward, but Employee #1 did not know the rod was moving upward while he was loading the part. The rod pushed Employee #1's finger into the point of operation, amputating his left index finger. Employee #1 was taken to the hospital, where he was treated for his injury. The two-palm buttons were merely a tripping device, which were not interlocked with the machine operation. The palm-buttons were not located far enough from the point-of-operation to prevent an injury.
