Employee'S Ankle Is Fractured In Fall From Ladder

At approximately 2:30 p.m. on April 27, 2006, Employee #1, working for a temporary agency, was doing some painting work on the walls of warehouse on a 2-section metal extension ladder. The ladder was not tied to the wall nor secured in any other way to prevent slipping. The floor was wet and slippery due to water and juice spillage inside the warehouse. The ladder was initially held by a coworker, while Employee #1 was going up, but after that, the coworker left the ladder standing against the wall without any extra support. The ladder slipped, and Employee #1 fell approximately 9 ft with the ladder and fractured his right ankle. Employee #1 was hospitalized.
