Employee Amputates and Lacerates Fingers Cutting Counters

At approximately 12:40 p.m. on July 19, 2006, Employees #1 and #2 were working at Doan's Formica Design, Inc., in Industry, CA. They were working on some built-to-order laminated commercial counters for a hair salon. Employee #1 was operating a 10-inch tilting arbor saw/table saw (a Delta Unisaw, with Catalog Number CAT 34-B06 and Serial Number 94B96630) to cut sheets of Formica 74 inches long, 6 inches wide, and one-thirty-second inches thick to different sizes. Employee #2 would then laminate these Formica sheets onto wood surfaces and assemble them into the final products. When the accident occurred, Employee #1 was cutting a 2-inch-wide Formica strip from a big piece of Formica sheet. He was holding the sheet with his left hand, and using a push stick to push the sheet forward to the cutting blade. After he cut the first strip, he pulled the rest of the sheet back. Unexpectedly, the sheet contacted the cutting blade and drew his left hand into the cutting zone. He amputated the third and fourth fingers of his left hand and lacerated the thumb and index finger of that hand. He was hospitalized.
