Investigation Summary
At approximately 4:00 p.m. on January 23, 2006, two mechanics, Employees #1 and #2, from Mechanic's Trailer and Lift Gate Repair Inc., arrived at Classic Distribution Company to repair industrial trucks. Classic Distribution located in Commerce, California was a beverage distribution company. On the day of the accident, Employees #1 and #2 were to perform repairs and replace the old axle of a beverage delivery truck, a 1990 Mickey, Serial Number LH043242, License Number 4EN2269, and Trailer Number 522. The old axle was removed, and Employee #2 proceeded to lift the trailer with a bottle jack in order to install the new axle. The mechanics wanted to attach two U-bolts, but the bottle jack and metal stand slipped and the trailer fell. When the trailer fell, Employee #1 suffered a hip fracture and was hospitalized in USC Medical Center.
