Employee Jumps from Forklift and Is Injured

At approximately 8:06 a.m. on January 20, 2006, Employee #1 was operating a fork lift in a storage warehouse. He was moving four large boxes stacked on top of each other, approximately 28 ft high. The top box was off center and was caught by a column supporting the roof trusses. The box became dislodged and fell, hitting the top of the forklift operator's roll-over protection. Just as the box fell, Employee #1, not wearing his seat belt, jumped from the seat. He was struck by falling materials. Employee #1 was hospitalized with unknown injuries.
