Investigation Summary
At approximately 1:45 p.m. on December 14, 2005, Employee #1 was assembling plastic parts on a 75 psi pneumatic press machine, when the die of press machine crushed her left middle finger. The pneumatic press machine did not have a point of operation guard, a cover on the machine, or a two-hand control safety feature. Employee # 1 was taken to a medical center. At the hospital, she underwent surgical procedures to repair the damages sustained in the accident. Following the surgical procedures, she was hospitalized for postoperative care, lasting one night.
