Investigation Summary
At approximately 3:48 p.m. on November 30, 2005, Employee #1, a warehouseman for a wire products warehouse, was manually lifting small wire rolls onto the forks of a parked forklift. During this process, a big wire roll, sitting in an upright position against a vertical support column adjacent to the small rolls, became unstable and fell from its upright position onto Employee #1. The roll that fell weighed approximately 700 pounds, with a 5 ft diameter. Employee #1 fractured his right leg. He was transported to Presbyterian Community Hospital in Whittier, California, and was hospitalized for four days. Employee #1 was not wearing steel-toed shoes at the time of the accident.
