Investigation Summary
On November 26, 2003, Employees #1, #2 and #3 were at the south end of a building unloading a 4-ft by 8-ft window panel from an extensible articulating boom. A window panel fell out of an aluminum frame and struck Employee #1 on the back of his neck. The window then fell forward striking Employees #2 and #3, causing a neck fracture to Employee #2, and a head injury to Employee #3. Employee #2 was hospitalized.
