Employee dies from a seizure

On November 4, 2003 on the third shift, Employee #1 was sitting on a manually propelled tricycle, Worksman Mover, with a tool box sitting on the back. He stopped and was talking with coworkers near a machine used to raise and lower a parts rack, so the operator does not have to stoop over. On this ergo lift is a ramp that changes angle when the ergo lift is raised or lowered. Employee #1 had the front wheel of the tricycle on the ramp and was rocking the wheel up and down the ramp. The machine operator raised the ramp to the maximum height, approximately 35 degrees. The tricycle flipped backwards with the front wheel going up and over the rear axle. The tool box and Employee #1 fell to the concrete floor. Employee #1 hit the back of his head on the floor. He got up and picked up the tool box and put it back on the rear of the tricycle. Coworkers asked if he was alright and he insisted that he was and rode away. Approximately 15 minutes later, he went to the restroom and had a grand mal seizure and died.
