Investigation Summary
At approximately 10.15 p.m. on October 19, 2006, Employee #1, a welder's assistant, was working with a coworker removing an 18-ft steel brace. The Company specializes in the design and fabrication of trailers used to transport unusual oversize objects such as houses and tanks. The steel brace was welded to a steel beam to ensure that the radius was maintained until it was bolted to the side frame rails of the trailer. The steel beam was bolted to the rail, and the welder had already cut and freed one side of the brace with his cutting torch. Employee #1 was standing on a 10 ft fiberglass A-frame ladder. He began to cut the steel brace on the other side with the cutting torch. When the brace was free, it fell onto the ladder, knocking Employee #1 off the ladder approximately 7 ft to the asphalt pavement. Employee #1 suffered injuries to his face and left hand and was hospitalized. He required surgery and therapy to regain the use of his left thumb.
