Investigation Summary
At approximately 4:44 a.m. on August 8, 2006, Employee #1 was working as a yard truck driver for St. George Warehouse, a container freight station and trucking company. He was backing an Ottawa Commando Model Number TJ508 truck toward a loading dock that was on an incline. Employee #1 stopped the truck approximately four feet away from the rear of the loading dock, after making contact with the rubber stops. He exited the truck to open the rear doors of the 40-foot container, but he did not engage the brake. The cab unit had no emergency brake. The truck moved backward, pinning Employee #1 between the rear of the vehicle and the loading dock. He sustained unspecified injuries to multiple parts of his body. He was transferred to Harbor UCLA Medical Center, where he died.
