Employee's Finger Is Amputated by Falling Equipment

At approximately 10:30 a.m. on June 19, 2006, Employee #1, a maintenance mechanic with Alpert & Alpert Iron & Metal, Inc. (a company that purchases and sells recycled materials to wholesale buyers), was working with a coworker in a maintenance shop to remove the radiator from a Freightliner tractor-trailer truck. In order to lift the radiator from the truck, Employee #1 used three automotive engine belts, linked together, to connect the top portion of the radiator to one fork of a Crown industrial truck (Serial Number 355-10514382). He asked the coworker to help him by operating the fork truck to lift the radiator. After lifting the radiator a few inches, it became stuck on the mounting brackets. Employee #1 then asked the coworker to lower the forks a small distance in order to free the radiator. After he did so, Employee #1 noticed that there was slack in the engine belts. When Employee #1 attempted to move the radiator with his hands, it fell and crushed his finger between the radiator and one of the engine belts. The finger was amputated.
