Employee's Finger Amputated When Crushed by Tanker Cover

At approximately 10:30 a.m. on May 12, 2006, Employee #1, a truck driver, was delivering environmental waste in a vacuum trailer truck with license Number T308568 and equipment Number 16006T. He was opening the 40-pound cast-iron cover, approximately 17 to 18 inches in diameter, on the dome of the vacuum trailer truck for a sample to be taken for laboratory test. The iron cover slipped out of his hand and he involuntarily tried to catch it with his right hand. His right ring finger was caught and pinched between the cover and the dome of the trailer truck. He sustained an amputation of about half an inch of his right ring finger and was hospitalized.
