Employee's Fingers Amputated in Pinch Point of Press

On May 12, 2006, Employee #1 was operating a Heim press brake. The direction of the flywheel was determined to have been rotating in a clockwise direction when the press brake was on the production mode. The flywheel of the press brake was thought to have been originally designed to rotate in the counter clockwise direction. The direction of the flywheel was suspected to have been reversed when a programmable logic control was retrofitted to the press brake early that year. The press brake was properly guarded by a presence-sensing device to prevent any inadvertent hand movement into the point of operation while the machine was in production cycle. It was determined the brake disc's retaining pin broke the disc into two pieces causing the brake to fail. The ram came down; free falling, pinching Employee #1's right hand while he attempted to retrieve a work piece from the point of operation. Employee #1 was hospitalized with an amputated finger.
